Rapid home-based occupational therapy when timely support matters

Care with Unity is a rapid, home-based occupational therapy service for people who need prompt assessment and practical solutions to stay safe and supported at home.

This service is designed to bridge the gap created by long waitlists and limited availability in publicly funded OT pathways. We already support many people referred by local GPs, hospitals, and community teams across Brisbane, and Care with Unity allows us to respond quickly when delays are not an option.

Our focus is on real-world problem solving in the person’s own home, so that risks are reduced, carers are supported, and unnecessary hospital admissions are avoided.

What we do?

With Care with Unity, an experienced occupational therapist attends promptly in the patient’s home (Brisbane metro) and provides:
  • Assessment in the real environment, including:
    • Safety and falls risk
    • Mobility and transfers
    • Bathroom access and toileting
    • Bed, seating and pressure care needs
    • Functional tasks and carer demands
  • A summary with clear, practical recommendations is provided after the appointment.

  • After the initial visit we can offer additional coordination once families decide how they’d like to proceed. We can support the provision of:
    • Commodes and shower equipment
    • Hospital beds and pressure-relieving equipment
    • Manual and powered wheelchairs
    • Ramps and grab-rails
    • Other minor home modifications
  • Ongoing support and coordination services are billed at $240 per hour

Pricing

Care with Unity is a private-paying service. The initial package is $440 which includes:
  • Travel to and from the home.
  • A focused in-home assessment with practical options/solutions discussed to help families understand what is available to assist.
  • A clear written action plan summarising needs, risks, and recommended solutions.

Ongoing appointments and support are available at an hourly rate of $240.
